Family Searches

If you are seeking information held by Corrective Services NSW (CSNSW) about a deceased family member, you can lodge an access application under the Government Information (Public Access) Act 2009 (GIPA Act).

An application form is not required to be completed. The application can be made in the form of a letter. It must be stated in the letter that it is an access application under the Government Information (Public Access) Act 2009.

A copy of the death certificate and documentation showing that you are the next-of-kin will be required. In addition, you will need to provide proof of identity. Usually, that is a copy of your driver licence. All the aforementioned documentation will have to be sighted by a staff member of CSNSW or certified by a Justice of the Peace.

The application fee and the processing charges may be waived. It is suggested that you discuss your application beforehand with a staff member from the Information Access & Privacy Unit.

If an application fee is not required, the application may be faxed to (02) 8346 1976 or emailed to infoandprivacy@dcs.nsw.gov.au.
